Apply for the Catalyst Award

Catalyst accepts applications from all organizations.  Each initiative is evaluated against the following criteria:

  • Business Rationale
  • Senior Leadership Support
  • Accountability
  • Communication
  • Replicability
  • Originality
  • Measurable Results

Members of the Catalyst Award Evaluation Committee assess each initiative through a rigorous and thorough evaluation process.  The Committee determines a select number of finalists, which it studies through interviews and focus groups during intensive two-day on-site visits.

The forms below comprise a completed application packet. Nominating organizations must submit the completed, hard-copy CEO/Managing Partner Attendance Verification Form and Nomination Fee Form via mail. The 2009 Catalyst Award application can be submitted via mail or email.
